Job Description

GENERAL SUMMARY:

Provides leadership in the coordination and delivery of quality compassionate patient care. Utilizes the nursing process of assessment, planning, implementation, and evaluation to provide, delegate, supervise and document care and teaching of patients and family. Effectively communicates with the health care team. Demonstrates clinical competence.

May require a professional certification which is a regulatory requirement in the State of Michigan or a documented community standard for the position, works independently and in collaboration with principal investigator when appropriate.

The position will serve two important roles: 1) Clinical Response to Patients' Elevated Patient Reported Outcomes (PROs) Scores, and 2) Clinical Care in OncoStat - our cancer urgent care program. Work schedules will be constructed to divide time appropriately between both roles. Over time, the proportion of time spent in one area or the other may evolve. There will be ongoing conversations to balance work schedules and priorities to ensure that PROs needs are addressed, and that OncoStat clinic coverage needs are met.

Patient Reported Outcomes Coordinating Center

* Provide clinical response to severe PROs scores by evaluating patients' scores, triaging, and contacting patients to assess needs and coordinate services:

  • Efficiently manage the Epic in-basket of severe scores received, such as Pain Interference, Physical Function, and other domains as needed.
  • Manage patient protocols and associated patient care in select PROs-related clinical trials or pilot projects.
  • Promptly contact patients reporting severe scores within one business day, conducting symptom triage, and determining appropriateness for referrals to OncoStat or other support services.
  • Facilitate referrals to OncoStat and to other support services, such as Pain Medicine, Psych Oncology, or others, guided by patient need and clinical assessments.
  • * Foster collaboration with OncoStat team members, Quality Coordinator, and cross-functional departments to anticipate and respond to patient needs proactively.
  • * Align efforts with the PROs Coordinating Center and Center for Patient Reported Outcomes (CPRO) to execute system and departmental initiatives.

OncoStat Program - Clinical Support

  • Conduct centralized telephone triage to assess patient symptoms and recommend appropriate clinical intervention for all patients across the Cancer Service Line.
  • Use Epic to schedule in-person or virtual appointments based on patient need and clinical assessment.
  • Deliver prompt, same-day symptom care to OncoStat patients, in-person or virtually.
  • Be a part of first response team for Code Blues in the Brigitte Harris Cancer Pavilion when working in the OncoStat clinic.

EDUCATION/EXPERIENCE REQUIRED:

  • Graduate from an accredited school of nursing.
  • Bachelor's Degree preferred.
  • Successful completion of henry Ford Ambulatory orientation upon hiring.
  • Maintains and demonstrates current clinical knowledge in area of specialty.
  • Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
  • Ability to perform a wide variety of professional nursing services.
  • Ability to effectively communicate with patients, families, and medical group personnel.
  • Preferred experience in the specialty.
  • Basic computer knowledge and keyboarding skills.

CERTIFICATIONS/LICENSURES REQUIRED:

  • Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
  • Current BLS or ACLS per Henry Ford Health policy.
  • Certification will be required when it is a regulatory requirement in the State of Michigan or a documented community standard, i.e., Oncology Certified Nurse (OCN).
