The Care Coordinator is responsible for providing outreach, disease management/case management, education/training and motivational support to INTEGRIS Health Partners covered lives to encourage healthy lifestyle choices and reduce long term effects of chronic illness. * RN, BSN Preferred * 3 years clinical experience * Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state * Previous work experience with educating patients and patient goal setting preferred
